When you purchase a device with a mail-in rebate offer, it’s important to submit the completed rebate form along with the requested documentation in the provided timeframe in order to receive your pre-loaded reward card. All of the information you need is on the rebate form.

In addition to the rebate form, you must also submit a dated store receipt (original or photocopy) for the device purchase.

The device must be active on your account for 120 days to be eligible for the mail-in rebate.

Meeting these conditions will make timely receipt of your reward card possible:

• Eligible Purchase Dates – These are the dates between which the device must have been purchased.
• Documentation Postmarked By – Your rebate form and dated store receipt must be postmarked by this date.
• Documentation Received By – Your rebate form and dated store receipt must be received at the address on the rebate form by this date.
